When applying for the following licenses you may obtain an application from the local Registrar’s Office, the Borough Website, or the link below.

Do not sign the application form before going to the Local Registrar. You must sign the application, while under oath, in the presence of the issuing authority.

Certified Copy of Vital Record

All requests for birth, death, or marriage certificates must be either mailed to the Borough of Mountainside, 1385 Route 22 Mountainside, NJ 07092 or in person.

  • IN PERSON:
    Original documents must be presented. Payment must be in the form of a check made out to the Borough of Mountainside or cash in the amount of $15.00 per copy.
  • BY MAIL:
    Photocopies are acceptable. Payment must be in the form of a check made out to the Borough of Mountainside in the amount of $15.00 per copy.
Acceptable Forms of Identification  
   
    • Valid photo driver’s license or photo non‐driver’s license with current address

OR

    • Valid driver’s license without photo and an alternate form of identification with current address.

OR

  • Two alternate forms of identification, one of which must show the current address.
 

People who are homeless can provide identification from a social worker or homeless shelter coordinator. People who are incarcerated can provide legal imprisonment, conviction papers, or release documents that include the name, social security number, and all possible aliases use in the past or identification from a prison/probation official.

Borough Clerk

The Borough Clerk serves as the secretary of the municipal corporation, as the local election administrator and as secretary to the governing body, providing a variety of administrative and community relations services for them. This office receives, distributes and assigns correspondence on behalf of the Mayor and Council, prepares Borough Meeting agendas, resolutions, proclamations, ordinances and public meeting minutes. The Clerk administers the bidding and awarding of municipal contracts. The Clerk also oversees a variety of licenses.

Election & Voter Registration

  • WHEN ARE THE PRIMARY AND GENERAL ELECTIONS?

    The Primary Election is held on the first Tuesday after the first Monday in June. The General Election is held on the first Tuesday after the first Monday in November.

  • WHAT TIMES ARE THE POLLS OPEN?

    6 am - 8 pm

  • I WANT TO REGISTER TO VOTE. WHAT GUIDELINES MUST I FOLLOW?

    You must be 18 years of age by the election. You must reside in Mountainside Borough at least 30 days prior to the election. You must be a Citizen of the United States.

  • WHEN MUST I REGISTER TO VOTE TO BE ELIGIBLE TO VOTE IN THE NEXT ELECTION?

    You must register any time prior to the 21st day before any election to be eligible to vote in that and subsequent elections.

  • WHERE DO I GO TO REGISTER TO VOTE?

    A voter registration form can be obtained at the followling locations; Mountainside Borough Clerk’s Office, at the Union County Board of Elections, or at your local Motor Vehicle Office. For other polling facilities click here.

  • ABSENTEE BALLOTS

    In New Jersey, any voter can now vote by Absentee Ballot for any election. You do not need a reason for an Absentee Ballot. A voter may apply for an Absentee Ballot by completing an Absentee Ballot application and mailing the application to the County Clerk by mail up to 7 days prior to the election. A voter may also apply in person to the County Clerk until 3:00 pm the day before the election. The County Clerk cannot accept faxed copies of an Absentee Ballot application since an original signature is required.

  • WHEN IS THE LAST DAY TO APPLY FOR AN ABSENTEE BALLOT?

    If mailed, no later than 7 days before the Election  If delivered in person to the County Clerk’s Office, no later than 3:00 P.M. the day before the Election.

  • MILITARY ABSENTEE BALLOTS:

    Military personnel and dependents may vote by military absentee ballot if: You are in the Military Service You are a spouse or dependent of someone in the military, you are a patient in a VA Hospital, or you are a civilian attached to the armed services. 

  • WHERE DO I VOTE?

    Please refer to the sample ballot which the County Clerk’s Office mails to you, contact the Borough Clerk’s Office, or check the NJ State Election Website.

  • WILL I EVER BE REQUIRED TO RE-REGISTER?

    Voter registration in New Jersey is permanent except when the voter has moved from the county in which they are registered. A Voter who moves within the same election district remains registered and may be allowed to vote for two years. If a Voter moves within the County within 29 days preceding the election, they may vote at their newly designated polling place by means of Failsafe Ballot. You should contact the Union County Clerk for this information.

  • I CANNOT APPEAR IN PERSON TO REGISTER. CAN I REGISTER BY MAIL?

    Yes, by completing a mail-in registration form. An application can be obtained from either the Borough Clerk of the Union County Clerk.

  • HOW DO I CHANGE MY PARTY AFFILIATION?

    A Party Declaration Form must be filled out. (May be obtained from either the County Board of Elections or Borough Clerk). Party Declaration Forms must be received by the Union Board of Elections no later than 50 days prior to the Primary.
